Breast lift (mastopexy) with scaffold

Breast lift with scaffold it is a procedure dedicated to women who still have large breasts, have not lost their volume, but have lost their unity and have fallen due to feeding and the passage of time. Additionally, the skin of these patients is thin and shredded with numerous stretch marks. These patients still have nice and large breasts but in a bra. They don't want to enlarge them; they just want nice breasts back! Each treatment is preceded by appropriate planning of skin cuts. The treatment consists in removal of only the epidermis around the areola-nipple complex (NAC) and below it and in the lower poles of the breast, further the complete dissection of the breast gland from the adjacent skin and supporting / suspending the lower poles of the breast and at the same time the entire gland with a mesh and lining it medially to the fascia of the pectoral muscle and laterally to the dentate muscles. Above the raised gland, it closes the non-delaminated skin leaving an anchor-shaped scar. Said mesh acts as an inner bra. This procedure is done in general anesthesia.

The mesh is completely soluble and is made of a material that is produced by Escherichia coli bacteria, which are bacteria that live in our gut, and without them our bodies would not be able to live. The presence of this material in our body is to stimulate our body and the surrounding tissues to create collagen fibers that will hold the gland and prevent it from falling back down again. It will dissolve within 2 years.

Many surgeons in such cases propose to "support" the gland with an implant. I use the word "support" specifically in quotation marks. Because it is not really support, but it is the worst thing that can be offered to such a patient. The implant additionally adds volume and loads the breast. Larger and heavier breasts with poor skin quality fall even faster and in addition, we get breast deformities called bottoming out or waterfall deformity.

Consequences and possible complications:

  1. Swelling and bruising - this is a consequence, not a complication.
  2. Pain of varying intensity depends on the level of the patient's pain threshold, but not too strong.
  3. Asymmetry, we are not symmetrical although it seems to the layman that our body is symmetrical. Therefore, it is impossible to achieve perfect symmetry despite all efforts.
  4. Hematoma, requiring surgical intervention
  5. Infection of wound and breast tissue with wound dehiscence, prolonged healing, and subsequent wide scarring
  6. Normal, hypertrophic and keloid scars.
  7. The emergence of no internal seams through the skin.
  8. Separation of internal seams in the form of micro-rises.
  9. Necrosis of the skin, subcutaneous and glandular tissue.
  10. Necrosis of the areola and nipple.
  11. Internal scars distorting the breast.
  12. Bottoming out breast distortion (stretching of the skin at the lower pole and the entire gland at the bottom of the breast).
  13. Lymph cysts (seroma).
  14. Periodic or persistent pain.
  15. Lack of nipple sensation.
  16. Allergies to drugs used.
  17. Venous thrombosis with pulmonary embolism and death.
  18. Re-drooping of the breast (the use of a mesh is to prevent it from falling but does not give a 100% guarantee).
  19. No possibility of breastfeeding.
  20. Changing the shape of the breasts over time. The effect of the treatment and the shape of the breasts is not guaranteed forever, it will change from month to month and from year to year and depends on the type of tissue and genetic conditions.
  21. Unsatisfactory aesthetic effect or lack of the expected effect, which is only a subjective assessment of the patient.

For a period of 4 weeks, avoid physical exertion, especially when it involves the arms i.e., lifting or lifting the arms up. Professional work can be started if it is not manual work after 1-2 weeks from the surgery.
